Billing Questions

 

We accept Master Card, Visa, American Express, Discover, a personal check and cash.  Please bring your insurance card and a photo I.D. with you to each appointment.

 

If you have questions regarding your billing statement, please call us at (480) 981-2888 and choose option 6.  If you call outside of normal business hours, please leave a message and we will return your call promptly.
